School Based Health Centers (SBHC)
Envision New Mexico has been working with New Mexico's schools to improve the quality of healthcare delivered to children and adolescents since 2008. Typically the most medically underserved population in every state, New Mexico’s adolescents need physical and behavioral healthcare.
Envision offers a multi-year SBHC-QI process for primary care and behavioral health providers and SBHC staff to expand their knowledge base and add new patient skills. Through coaching and practice facilitation, SBHC staff implement changes in healthcare service delivery using quality improvement strategies and tools. The program is designed to help every professional develop the capacity to support healthy development and provide care to children and youth with medical and behavioral health needs.
